Cold Water Shock — Symptoms, What Happens, and What to Do
Cold water shock kills people every year in the UK. Not because they could not swim. Not because they were far from shore. Because the involuntary physiological response to sudden cold water immersion overwhelmed them before they had a chance to react. If you are offshore and need to get of a vessel, they use an Immersion suit to stop this and improve survival in an emergency.
Understanding what cold water shock actually does to your body is the knowledge that keeps you alive.
What Is Cold Water Shock?
Cold water shock is the involuntary physiological response to sudden immersion in cold water. It is not a choice. It is not something you can control through willpower or fitness. It happens to everyone in water below approximately 15 degrees Celsius — which covers UK coastal and fresh water for most of the year.

The Symptoms — What Happens to Your Body
The response happens in stages:
0 to 3 minutes — the cold shock response
Immediate gasping — an involuntary intake of breath that cannot be prevented. In choppy or rough water this gasp can cause you to inhale water before you have consciously registered what is happening. This is how people drown in cold water with no other injuries.
Hyperventilation — rapid, uncontrolled breathing that can cause dizziness, disorientation, and panic. You feel as though you cannot breathe even though you can.
Heart rate increases dramatically. Blood pressure spikes. In people with pre-existing cardiovascular conditions this phase can cause cardiac events.
This initial phase typically lasts 1 to 3 minutes. It is the most dangerous phase because it is when people attempt to swim and cannot control their breathing adequately to do so safely.
3 to 30 minutes — swimming failure
As cold water penetrates to the muscles — particularly the arms, hands, and legs — muscular performance deteriorates rapidly. Swimming ability can be lost within 30 minutes in cold UK water. In very cold water (below 10 degrees) this can happen significantly faster.
30 minutes plus — hypothermia
Core temperature begins to fall significantly. Confusion, drowsiness, loss of consciousness. This phase is slower than most people expect — the body maintains core temperature surprisingly well for 30 minutes or more.
What to Do If You Enter Cold Water Unexpectedly
Do not attempt to swim immediately. This is the most important instruction. The instinct is to swim hard for shore or the boat. Fighting the gasping, hyperventilating response while trying to swim hard is how people exhaust themselves and go under.
Float. The RNLI’s Float to Live response — spread your arms and legs, tilt your head back, keep your airway above the water. Let the initial cold shock response pass. This takes 60 to 90 seconds. Float through it.
Control your breathing. Breathe slowly and deliberately. The hyperventilation is an automatic response — conscious slow breathing counteracts it. Breathe in through the nose, out through the mouth.
Signal for help. Wave one arm. Shout. Activate your PLB. Call on your VHF.
Then swim if you need to — once the initial response has passed and you are calm, you can attempt to swim to safety or swim to your vessel if it is close. If you cannot reach safety — continue floating and wait for rescue.
What to Do If Someone Else Goes In
Do not jump in after them unless you are a trained lifesaver. This causes double drownings regularly. Shout instructions — float, do not swim, we are calling for help. Call 999 for the Coastguard immediately. Throw anything that floats. Keep talking to them.
If they lose consciousness — they need to be kept face-up. A lifejacket does this automatically. Without one, keeping their face above water is the only priority.
After Rescue — The Afterdrop
Once someone is out of the water — core temperature continues to drop for 20 to 30 minutes as cold blood from the extremities circulates back to the core. A person who seems fine on rescue can deteriorate significantly 20 minutes later.
Remove wet clothing. Insulate from wind. Warm drinks if conscious. Medical assessment for anyone who was in cold water for more than a few minutes.
FAQs
At what temperature does cold water shock occur?
Below approximately 15 degrees Celsius for most people. UK sea temperatures are in the range where cold water shock can occur for most of the year. Even in the warmest months in southern UK waters — 17 to 18 degrees — some cold shock response will occur.
Does a wetsuit prevent cold water shock?
A wetsuit significantly reduces the cold shock response by slowing the rate at which cold water reaches the skin. It does not eliminate it entirely but reduces both the severity and duration of the initial response significantly.
Can fit, healthy people die from cold water shock?
Yes. Cold water shock is a physiological response independent of fitness, swimming ability, or health. The gasping reflex and cardiovascular response can affect anyone. Fitness does not protect against cold water shock — preparation, knowledge, and the Float to Live response do.
