Cold Water Shock — What It Is and How to Survive It
I have experienced cold water shock a lot. As a scuba diver and open water swimmer I know exactly what it feels like — I’m sure you have experienced it in som regard but when its extreme cold, I can tell you it is nothing like people imagine until it happens to them.
The worst I have experienced was diving in Scapa Flow in the Orkneys in a drysuit with only a little bit of my face exposed. The moment the cold water hit the skin around my mask it felt like being stabbed. Not a metaphor — that is genuinely the sensation. Your body does not ease into it. It hits you instantly and completely.
Here is what is actually happening, and more importantly, what to do about it.
What Is Cold Water Shock?
Cold water shock is a sudden and involuntary response that occurs when the body is unexpectedly immersed in water below 15°C. Even on a hot day, the sea, rivers, and lakes in the UK can remain cold enough to trigger this reaction. Royal Yachting Association
When you first enter cold water, your body reacts instantly. The sudden cooling of the skin causes an involuntary gasp, which can lead to inhaling water. Your breathing rate increases rapidly, and your heart rate spikes. This can cause panic, disorientation, and even hyperventilation, making it difficult or impossible to swim. In plain English — your body panics before your brain has a chance to tell it not to. That is why people drown in cold water even when they are strong swimmers. It is not about fitness or ability. It happens to everyone.

What Does It Actually Feel Like?
That involuntary gasp is real and it is uncontrollable — at least the first time. Depending on the water temperature it hits differently. In truly cold water like the North Sea in winter, the sensation is immediate and overwhelming. In milder conditions it is still a shock but more manageable.
Your instinct will be to thrash, to swim hard, to fight it. Do not. That instinct will drown you faster than the cold will, a life jacket or immersion suit will support you in both floating and relaxing and keeping warm.
The good news: the RNLI’s advice is to float for around 60 to 90 seconds — the time it takes for the effects of cold water shock to pass and for you to regain control of your breathing. nih
Sixty to ninety seconds. That is all. If you can get through that window without inhaling water, your chances of survival improve dramatically.
My Tip From an Open Water Swimmer
A Swedish friend who swims in cold water regularly shared advice I now pass on to everyone: get in slowly if you possibly can. Wade in gradually. Give your body a moment to adjust rather than hitting it all at once.
In an emergency situation where you have gone overboard unexpectedly, you cannot choose your entry — but in any situation where you have a choice, a slow entry gives your body a fighting chance to adapt before the shock response overwhelms you.
The Float to Live Technique
The RNLI suggests that the best response is to float on your back until you get control of your breathing. This puts you in a better position to plan your next move, increasing your odds of survival in an extremely dangerous situation. Met Office
In practice: lean back, extend your arms, keep your face above water, and do nothing active. Big slow breaths. Do not put your face underwater yet. Do not try to swim. Just float and breathe.
It sounds simple. In cold water with your heart hammering and your lungs refusing to cooperate, it is one of the hardest things you will ever make yourself do. But it works.
Using the RNLI’s float advice, a 24-year-old man was able to stay alive in the River Thames for 25 minutes until he was rescued. He remembered the advice he had seen online. It saved his life. Met Office
The Splash Guard — Why It Matters
If you are wearing an immersion suit, make absolutely sure your face splash guard is in position before you enter the water. The involuntary gasp of cold water shock combined with a wave breaking over an unprotected face is a drowning risk. The splash guard exists precisely for this moment.
Pinch your nose as you enter the water, keep your mouth closed, brace for the cold, and get that splash guard up the moment you are in.
Your Step by Step Survival Plan
- PUT ON A LIFE JACKET IF POSSIBLE : Brace for entry — pinch nose, close mouth, brace
- Do not panic — easier said than done but critical
- Float on your back immediately — arms and legs out, face up
- Breathe slowly and deliberately — in through the nose, out through the mouth
- Wait 60-90 seconds for the shock response to pass
- Only then — assess your situation, look for something to hold, signal for help
- Conserve energy — do not swim unless safety is very close
One More Thing
Symptoms of cold water shock to look out for are coughing, chest pain, troubled breathing, tiredness, and irritability. It can also induce vertigo as your ears are exposed to cold water, resulting in failure to differentiate between up and down when submerged.
If you recover someone from the water who seems fine — do not leave them alone. Cold water shock and the effects of hypothermia can be delayed. Keep them warm, keep them monitored, and get medical attention.
